Thank you for choosing to be a Team Captain for the 2017 Buddy Walk.

Team Information Form
After setting up your team and registering for the walk please complete the
Team Information Form to help us with planning for the event. This form includes team tent space request, buddy t-shirt size information, and celebration poster information.
Donate a team basket to be raffled off at the Buddy Walk. The basket can be themed or not, big or small, unique or popular. Just drop off your basket – labeled with your team name – at the event site during set-up on Friday May 12th from 3pm-7pm or before 11am on Saturday May 13th. Raffle Ideas and Tag

Teams can solicit local companies or businesses to become a T-shirt Sponsor and your team will get the credit to their team total. For $500 the company logo can appear on the back of the Buddy Walk Event T-shirts or for $250 the company name can be listed on the back below the logos.
T-shirt sponsors must submit their logo or name with payment, no later than April 12th.
